Surgical Techniques -Textbook for medical students

“Surgical techniques” is a subject relating to the principles of surgery. Traditionally it discusses instrumentation, tools and sterile techniques used to perform safe operations by generations of surgeons. A relatively new, but continuously expanding element is linked to the development of clinical surgery. This part involves surgical research on the pathophysiology of the perioperative period, and scientific studies on the consequences of invasive procedures on the body functions and on the relationship of surgery and patient care in general. These aspects are of utmost importance for the achievement of good surgical results. In brief, “surgical techniques” is concerned with the methodology of classical surgery, and may also be regarded as the deontology of modern surgery (deontology = theory of obligations). This handbook is based on the topics presented on the graduate courses at the Institute of Surgical Research at the University of Szeged. These courses do not cover all aspects of clinical surgery; the intention is rather to give a general overview of basic techniques which could be used in practice in all major fields and subspecialties. There are a number of conventional surgical techniques that have been in use for some time and the scalpel is the symbolic instrument of these fields. However, it is being replaced by new and more effective surgical tools, sophisticated computer technology and techniques such as laser, robotic or minimally invasive surgery. If we wish to be at the forefront of these advances, the new methods should also be discussed.

Drug-Drug Interaction Primer: A Compendium of Case Vignettes for the Practicing Clinician

This user-friendly resource offers complete and comprehensive coverage of the difficult challenges posed by drug-drug interactions. Over 170 case vignettes illustrate a variety of interactions (DDIs) to provide an unintimidating even entertaining approach to understanding these issues. Drug-Drug Interaction Primer builds on the author's earlier work, Drug Interactions Casebook: The Cytochrome P450 System and Beyond, and features updated references throughout and 29 new cases that provide such clinical examples as: a patient diagnosed with schizoaffective disorder transitions from haloperidol to aripiprazole with disastrous results; an AIDS patient's trimethoprim-sulfamethoxazole is replaced with warfarin following atrial fibrillation, resulting in ischemic stroke; and a delusional patient on risperidone is placed on phenytoin following seizures, and delusions return. Previous cases have been revised to enhance clarity. An introduction to core concepts, which includes brief reviews of each enzyme system, brings the reader up to speed on how to think about DDIs and begin to grapple with what might seem like an imposing subject. The vignettes that follow each include a case presentation and an explanation of the mechanism by which the interaction(s) occurred, and each derives from sound clinical evidence not merely extrapolations from drug characteristics to offer a more realistic understanding of DDIs. Most of the interactions described involve the cytochrome P450 enzyme system; others involve alterations in phase II metabolism and P-glycoprotein functioning, as well as plasma protein displacement effects. The appendices detail most drug-drug interactions between psychotropic agents and contain metabolic pathways and inhibitory and inductive profiles for antidepressants, antipsychotics, and mood stabilizing agents as well as tables that detail all known and clinically significant DDIs between pairings of any two agents from these drug classes. An extensive index allows quick reference. Among the book's other features:
*Reorganization by medical subspecialty psychiatry, internal medicine, neurology, surgery/anesthesia, and gynecology, oncology, and dermatology better facilitates clinical application.
*Comprehensive tables detail substrates, inhibitors, and inducers for P450, phase II, and P-glycoprotein.
*DDIs involving select nonpsychotropic agents such as tobacco, ethinylestradiol, and statins.
*Exploration of the paradigm of plasma protein binding mediated DDIs in detail, with cases conveniently indexed.
These cases bring DDIs alive in a way that drier descriptions cannot, and this volume introduces more original material than will be found in other sources. Drug-Drug Interaction Primer is brimming with material that can be put to immediate use, offering insights that will improve any practitioner's skills.

Echocardiographic Anatomy in the Fetus

Whether in fetal or postnatal life, echocardiographic diagnosis is based on moving images. With recent advances in ultrasound systems, storing multiple digital frames and clips with superb image quality has become a reality. These advances have brought innovative applications into the clinical field and can be integrated into powerful multimedia presentations for teaching purposes.
Sections of cardiac specimens are usually compared with corresponding sections in echocardiography textbooks. These sections are mainly obtained from isolated hearts, due to ease and speed of acquisition. Nevertheless, sections of the whole body are a better tool with which to understand the relationship between cardiac and extracardiac structures. This understanding is particularly important in fetal echocardiography, where the number of visible structures around the heart is much greater and the approaches to the fetal thorax are more variable.
The DVD accompanying the book presents morphological images from tomographic sections of the whole fetal body, as well as high-quality dynamic echocardiographic images of normal fetuses and of some of the most common congenital heart defects.
The extraordinary amount of information provided in this work makes this publication a highly useful resource for obstetricians, sonographers, and pediatric cardiologists.
